Exercise and Type 2 Diabetes: How Moving Your Body Helps Control Blood Sugar

Regular movement is one of the most powerful tools we have for managing blood sugar, and in this post we'll break down exactly why, using plain language along the way.
First, a Quick Refresher: What's Actually Happening in Type 2 Diabetes?
When you eat, your body breaks food down into glucose (a type of sugar) that travels through your blood to fuel your cells. Normally, a hormone called insulin acts like a key, unlocking your cells so glucose can get in and be used for energy. In Type 2 diabetes, your cells become less responsive to insulin, a problem called insulin resistance, so glucose builds up in the bloodstream instead of being used properly. Over time, this leads to high blood sugar (also called hyperglycemia), which can damage blood vessels, nerves, and organs if it isn't managed well.
Exercise is powerful here because working muscles can pull glucose out of the bloodstream on their own, using a pathway that doesn't even need insulin to work. In other words, movement gives your body a second way to lower blood sugar, one that works even when insulin resistance is part of the picture.
Exercise Doesn't Just Help "In the Moment" — It Improves Long-Term Control Too
A comprehensive scientific consensus statement from the American College of Sports Medicine confirms that regular aerobic exercise (the kind that raises your heart rate, like walking, cycling, or swimming) meaningfully improves long-term blood sugar management in people with Type 2 diabetes. The most common way doctors track long-term blood sugar is a blood test called HbA1c, which reflects your average blood glucose over roughly the past three months. Think of it as a "report card" for your sugar control rather than a single day's snapshot. According to this expert panel, regular aerobic exercise training improves glycemic management in adults with Type 2 diabetes, with less daily time spent in high blood sugar and reductions in HbA1c of about 0.5 to 0.7 percentage points, a meaningful improvement by clinical standards.
Importantly, these benefits don't only show up in a lab. A large systematic review pooling data from 32 randomized controlled trials found that after a program of regular exercise, almost all the studies reported decreases in HbA1c, fasting blood glucose, body mass index, and waist circumference. On average, the people in these studies exercised for about 45 minutes per session, a little over three days a week, for close to 22 weeks, a realistic, sustainable routine, not an extreme one.
Strength Training Matters Just as Much as Cardio
It's easy to assume "exercise for diabetes" means cardio alone, but resistance training (lifting weights, using resistance bands, or working against your own body weight) plays a major role too. According to the ACSM consensus statement, resistance exercise training in adults with Type 2 diabetes typically results in 10 to 15% improvements in strength, bone density, blood pressure, blood lipid levels, muscle mass, and insulin sensitivity, insulin sensitivity being how responsive your cells are to insulin's signal to absorb glucose. Even better, when resistance training is combined with modest weight loss, it can reduce HbA1c three times more than calorie-cutting alone in older adults, without the muscle loss that often comes with dieting by itself.
Combining aerobic and resistance training tends to work even better than either one alone. One randomized trial found that a training program combining both types of exercise led to a bigger drop in HbA1c than either aerobic or resistance training by itself.
Short, Intense Bursts of Exercise Can Work Too
If a long workout feels daunting, there's good news: high-intensity interval training (HIIT), short bursts of harder effort mixed with rest, has also been shown to help. The ACSM panel notes that HIIT training significantly improves fitness and reduces HbA1c and body mass index in adults with Type 2 diabetes. Moreover, one study found that a single session of just ten sixty-second bursts of cycling at high effort reduced blood sugar spikes after meals in people with Type 2 diabetes. That said, HIIT isn't necessarily "better" for everyone, it can carry a slightly higher risk of minor injury, so it's worth easing into it and listening to your body.
Small Movements Throughout the Day Add Up
You don't need to save all your movement for a single workout session. Breaking up long periods of sitting with brief bouts of activity has a real, measurable effect on blood sugar too. Research reviewed by the ACSM panel found that interrupting prolonged sitting with light walking or simple resistance activities for just three minutes every half hour decreases blood sugar, insulin, and other markers after meals, and that replacing sitting time with standing and light walking throughout the day can improve glucose levels and insulin sensitivity even more than a single structured workout. In other words: taking the stairs, pacing during a phone call, or getting up every 30 minutes to stretch genuinely counts.
How Much Exercise Is Recommended?
The official guidelines summarized by the American College of Sports Medicine recommend that adults with Type 2 diabetes aim for 150 to 300 minutes of moderate-intensity aerobic activity per week (or 75 to 150 minutes of vigorous activity), spread across most days, with no more than two consecutive days without activity, plus resistance training on two to three non-consecutive days each week involving all the major muscle groups. That might sound like a lot, but broken down, it's roughly 30 minutes a day, five days a week, at least a couple of short strength sessions. And as the research above shows, even activity broken into small chunks throughout the day still counts toward these benefits.
Exercise Also Helps Beyond Blood Sugar
Type 2 diabetes rarely travels alone, it's often linked with other cardiovascular risk factors like high blood pressure and unhealthy cholesterol levels. The research consistently shows exercise helps here too. According to a broad review published in Nature Reviews Endocrinology, increases in physical activity and fitness are an important contributor to Type 2 diabetes improvement when combined with calorie management and weight loss. And a review focused specifically on the mechanisms behind exercise's benefits summarized it well: exercise training can favorably affect not just blood sugar, but also cholesterol levels, blood pressure, and markers of inflammation, while reducing cardiovascular risk factors and helping regulate body weight by reducing body fat and building lean muscle.
A 12-week clinical trial comparing aerobic-only, strength-only, and combined exercise programs in people with Type 2 diabetes makes this point vividly. All three exercise approaches meaningfully lowered fasting blood sugar, body fat, and both the top and bottom blood pressure numbers compared to a non-exercising control group. But, the combined aerobic-plus-strength group saw the biggest improvements across the board, with reductions in blood sugar, blood pressure, and body composition all significantly greater than either type of exercise alone. It's a good reminder that you don't have to choose one style of movement over another, mixing cardio and strength work may be the most effective approach of all.
A Few Safety Notes
Exercise is safe and beneficial for the vast majority of people with Type 2 diabetes, but a few precautions are worth knowing:
If you use insulin or certain other diabetes medications, exercise can occasionally cause blood sugar to drop too low (hypoglycemia), so it's worth discussing timing and snacks with your doctor.
If your blood sugar is very high before exercising, it's best to check with a healthcare provider about whether it's safe to proceed that day.
Anyone starting a new, more vigorous exercise routine, especially after being inactive, or with existing heart or diabetes-related complications, should get the green light from their doctor first.

Whether it's a brisk daily walk, a couple of strength sessions each week, short bursts of higher-intensity effort, or simply standing up more often throughout the day, the evidence is clear: movement is one of the most effective, accessible tools available for managing Type 2 diabetes. It works alongside medication and nutrition, not instead of them, and even modest, consistent exercise can make a measurable difference to long-term blood sugar control and overall health.
